Submit a Data Subject Request

You may have certain privacy rights depending on your country or state of residence. In order to submit a request for access (portability), deletion, or correction, please complete and submit the following form. After we receive your request, we will contact you to verify your identity and determine your eligibility, as required by applicable law. We may not honor your request if we are unable to verify your identity.

If you are an authorized agent trying to exercise rights on behalf of a consumer, please select the appropriate option below under Requestor Type, select the applicable jurisdiction under Applicable Jurisdiction, fill out your information, and upload supporting verification information which should include proof that you have the consumer’s authorization to act. Under certain circumstances, we may also require you to prove and verify your identity or the identity of the authorizing consumer if you are acting as an authorized agent on behalf of a consumer.
